Report Sexual Misconduct, Stalking and Domestic/Dating Violence
Use this form to report any possible violations of Florida State University's Sex Discrimination and Sexual Misconduct Policy such as all forms of non-consensual sexual activity and unwelcome sexual conduct including: sex discrimination, sexual violence (rape/sexual battery/sexual assault), sexual harassment, relationship violence (domestic violence and dating violence), stalking, harassment or violence based on sexual orientation, gender identity or gender expression, and other forms of sexually exploitative behavior.

To report alleged violations of the Student Conduct Code and/or Student Organization Conduct Code, please use the online reporting form available here. Reports are submitted to the Office of Student Conduct & Community Standards for review. Non–academic violations include any behavior that is not related to an allegation of cheating or plagiarism (e.g. alcohol, drugs, harassment, vandalism, disorderly conduct, etc.). If you have questions about whether a behavior is considered a violation, please contact the Office of Student Conduct and Community Standards.

Report Concerning Behavior
Though the term "concerning" is subjective, you may feel a student is exhibiting academic, emotional, or physical behaviors that are of concern. Typical examples include consistent unexplained absences, yelling or aggressive comments, or deteriorating physical appearance or personal hygiene. If you believe a student is displaying concerning behaviors, please report this information via our online form.

For students who live on-campus, University Housing staff takes the lead on processing incidents and concerns. Use this form if the incident or behavior impacts on-campus students or occurred in any University-owned Housing facility.

Report ADA/Accessibility Issues
At times the University may be unaware of ADA/accessibility issues on campus. Examples of ADA/accessibility issues includes, but is not limited to, obstructions or repairs needed to paths of travel, broken or malfunctioning elevators, push button malfunctions, and issues with access to a building. If you have identified a potential ADA/accessibility issue or concern, please report this information via our online form.

Florida State University is committed to creating and maintaining a safe and welcoming atmosphere for all community members. Fostering this climate includes taking constructive action if one witnesses or knows about behavior that violates the University's policy on discrimination. This process is designed to aid students in reporting and responding to acts of discrimination. Upon receiving the report a staff member will follow up and respond as appropriate.

Students are not to be demeaned or exposed to harm when pursuing involvement in campus life. Hazing activities create an unsafe campus environment and include, but are not limited to, forced or coerced consumption of alcohol, deprivation of food or sleep, beating or paddling in any form, personal servitude, or kidnapping or abandonment.

Click here to report alleged violations of the Student Organization Conduct Code. Potential violations include alcohol, drugs, bullying, harassment, vandalism, disorderly conduct, and open parties. Submitted reports are reviewed by the Office of Student Conduct and Community Standards and followed up on appropriately.

Refer a Student for Academic Skills Assistance
Use this form to refer a student for academic skills assistance. Reasons for using this form could include a student's lack of class attendance or participation, poor assignment/test grades, concerns about motivation, or any other general academic concern. The student will be contacted via the Proactive Referral and Engagement program and offered one-on-one assistance.

Report Behavioral Issues in Canvas Courses
FSU IT policies and guidelines strictly prohibits using a University account for spam including but limited to self-advertising (selling textbooks and notes, renting or leasing apartments, etc.). Each of your FSU-sponsored accounts is only to be used for academic, non-commercial purposes. This includes Canvas and FSU webmail. Use this form to report violations of this policy by students in your Canvas courses.

If you have been at an event or venue in Tallahassee and found an unexplained mark or paint on yourself and/or suspect that your drink may have been spiked, please file a report at report.fsu.edu via the new Drink Spiking button. If this has happened to someone you know, encourage them to file a report as well.

While we have the ability to accept anonymous reports, our ability to follow up and respond will be limited without identifying information.

If you would like to speak to a confidential resource for support or to discuss related concerns, please contact the Victim Advocate Program at (850) 644-7161, or their text line (850) 756-4320, 24/7. You may also report information directly to FSU Police Department if you wish to provide information about unexplained marks or paint on yourself and/or spiked drink concerns at (850) 644-1234.

 

Victim Advocate Program

Victim Advocate Program
FSU Victim Advocates provide support to victims of all crimes, including sexual assault, domestic violence, dating violence, and stalking. They are available 24 hours a day, 7 days a week. All Victim Advocate Program services are free, confidential, and available to any student, faculty, or staff member who is the victim of actual or threatened crime, anywhere, at any time (including childhood abuse). You do not have to file a police report or University complaint to receive support. In addition to this report, you can also call the Victim Advocates at 850-644-7161, or their text line 850-756-4320.
